J-1 Scholar Change of Address

This form is for International Scholars in J-1 visa status at FSU.

U.S. Citizens and Residents should go to http://campus.fsu.edu to report a change of address to the Registrar's Office.

Please make sure that the International Center has your correct contact information at all times. Under federal immigration regulations as well as University regulations, all international students must report a change of address within 10 days of the change. The International Center will in turn report the address change to the Department of Homeland Security thru the Student and Exchange Visitor Information System (SEVIS).

INSTRUCTIONS:

Report your change of address by completing both Step 1 and Step 2:

Log in to campus.fsu.edu (Blackboard)
Go to the “Secure Apps” tab on the upper right corner
Click on "My Address."
Update your Local or Permanent Address or even add an Emergency address. (For a list of the new street addresses for FSU Residence Halls, see FSU Residence Hall Street Addresses).

NOTE: The online form below should be used ONLY by students who have completed their degree and are have a pending or approved request for Optional Practical Training or Academic Training.
