Registration Requirement
Health insurance which covers one in the U.S. is required of everyone in F-1 status per state of Florida requirements. All International students will have to show proof of purchase of a health insurance policy which complies with the requirements listed below BEFORE the Thagard Health Center registration hold will be lifted from their account to allow for registration. Since the windows of registration begin opening early in the semester, it is important that you understand and comply with the requirements BEFORE your registration window opens. Do not wait until you try to register to begin exploring insurance policy options.
You must show proof of purchase of acceptable health insurance that covers you for 52 consecutive weeks beginning on the first day of class in August and continuing through the day following the last day of summer session*. The insurance needs to be purchased
and you need to submit proof of purchase to the Health Center Insurance office before you will be able to register for any classes.
- Note: Students starting at FSU in the Spring or Summer semester will need to provide proof of insurance from the first day of the first semester enrolled until the day following the last day of the last summer session. Then, the next year of coverage may start in August.
- Coverage Period: 52 continuous weeks of coverage.
- Basic Benefits: Room, board, hospital services, physician fees, surgeon fees, ambulance, outpatient services, and outpatient customary fees paid at 80% of usual, customary, reasonable (UCR) charges after deductible is met.
- Inpatient Mental Health Care: 50% of the usual and customary fees with a minimum $100 cap.
- Outpatient Mental Health Care: Paid at 50% of the usual and customary fees with a minimum $100 cap.
- Maternity Benefits: Treated as any other temporary medical condition.
- Inpatient/Outpatient Prescription Medication: Offers coverage.
- Repatriation: $10,000 (coverage to return the student's remains to native country).
- Evacuation: $10,000 (permits the patient to be accompanied by a provider or escort if directed by the physician in charge).
- Exclusion for Pre-Existing Conditions: First six months.
- Deductible: $50 per occurrence if treatment is rendered at a State University Student Health Center, $100 per occurrence if treatment is not rendered at the Student Health Center.
- Minimum Coverage: $200,000 for covered injuries/illnesses per individual student.
- Insurance policy and premium rate have Florida Department of Insurance Approval.
- International students have two options for meeting the insurance requirement.
