OPT Application for 17-Month Extension Only
NOTE: The online form below should be used ONLY by students who have completed their degree and have been authorized for Optional Practical Training.
OPT Extenson Application Instructions:
Please read all the instructions carefully.
- Read the Information Brochure on the 17 month OPT Extension.
(Note: Florida State University Human Resources confirmed that FSU has opted NOT to participate in E-Verify at this time. The University will continue to monitor developments and improvements in the E-Verify system.)- If you meet eligibility requirements, and if your Employer is enrolled in E-Verify, talk to your Employer about your application for extension.
- Provide the Employer with the Employer Brochure, and Employer Form for Application for OPT Extension.
- Complete the online Application for 17 Month Extension form and submit it electronically. The link is provided at the end of this page.
- Submit the material below to the International Center. See the box below for instructions on how to submit these supporting documents.*
- Form I-765
- Acknowledgment of Responsibilities During OPT
- Employer Form for Application for OPT Extension
- Copies of most recent immigration documents:
- Copy of your I-94 card, front and back
- Copy of your EAD, front and back
- Copy of your passport biographical pages (pages containing the passport number, expiration, birth date, and picture)
- Copy of your F-1 visa
- Self-Addressed, prepaid mailing envelope (USPS Priority or Express Mail, Fedex or other Courier) so that we can email you back your application packet. We will provide additional instructions in this packet.
The following documents do NOT need to be sent to the IC; however, you will need to include them in the application packet that you will receive back from the IC for mailing to USCIS: - Proof of the Degree and Major such as:
- Official/Unofficial transcripts, or
- Copy of the diploma
- Two identical color photographs of yourself taken within 30 days of the filing of this application.
- Filing fee: Check or money order for $340.00
- You will receive your processed application packet back from your IC adviser with the new I-20 recommending the extension. Instructions (with Service Center addresses) and a checklist will be included in your packet. You will mail your Completed Application to the appropriate USCIS Service Center.
- Email, fax or mail a copy of your signed I-20 back to your IC adviser.
