Students Covered by Home Country or Third Party
- 1If your home country, or a third party, such as LASPAU, IIE, Ford Foundation, is responsible for paying for your insurance, you will need to complete a third party billing form available at www.tshc.fsu.edu/intemational.htm and present it to the Thagard Insurance office along with a copy of your sponsorship letter.
OR - If your sponsor has already purchased Insurance for you, you will need to follow the instructions under Option 2 for comparable insurance. If the insurance does not meet all the requirements, your sponsor will be required to purchase additional coverage for you in order to comply with State requirements for international students.
Once the documentation described above is received by Thagard Student Health Center, your registration hold will be removed. If applicable, the country or third party indicated on your third party billing form will be billed for the university sponsored insurance plan. If you DO NOT complete either form above, your registration hold WILL NOT be removed.
